This form, along with a consultation with a Fat Kitty City Humane Society representative, are designed to help us determine if the adoption is in the animal's best interest and to assist you in finding an animal most compatible with your lifestyle.
In order to be considered as an adopter you must:
Be 21 years of age or older
Have government-issued identification showing your present address
Have the knowledge and consent of your landlord
Be able and willing to spend the time and money necessary to provide training, medical treatment and proper care for a pet.
Completion and submission of the Adoption Form does not guarantee adoption of an animal from the Fat Kitty City Humane Society.

Fat Kitty City Humane Society is a non-profit 501(c)3 organization dedicated to ending needless suffering, through spaying, neutering, socialization & adoption, and other community-based programs. We currently operate a rare on-of-a-kind no-kill cat sanctuary on a 20-acre tree-studded natural environment where cats can live safely and roam free in Northern California.
